Proposed Lock out Tag out Procedure

 

In an emergency an engineer will immediately put the device into a safe position.  At least some type of temporary tag should be put in place as soon as possible if there is no supervisor available.  At the earliest possible time a supervisor needs to be advised and ensure it is tagged properly and all documentation is in place. 

 

 

Example procedure:

Upon request work and authorization by a supervisor the engineer will position the devices and tag the equipment.  For each tag the number will go sequentially i.e. 1A, 1B, 1C, 1D…  No un-tagging is allowed unless all equipment can go to the ‘Un Tagged’ position.  If any of the tagged equipment needs to be repositioned and all equipment is not back to 100% a new tag number must be issued.  The old tag must be cleared at that time.  This will prevent having to look through ½ closed tickets and determine what is still open. 

 

The ‘Un Tag Code’ codes will be as follows:

G-General, E-Electrical, M-Mechanical

 

There will be a list in the front of the log book of the people under each code.  Supervisors will decide who fits under what code.

 

The locks, tags, and Log will be kept in the top drawer of the cabinet that the clam shell is kept in.   

A new location needs to be chosen for the Log. 

 

1)Date – The date the event started.

2) Tag # - The next sequential number.

3) Description of Work – A detailed description of the work to be preformed.

4) Date Completed – The date any part of it is complete.  If part is complete close out and start a new ticket with the remaining items.

5) Actions to be taken -

6) Equipment – What parts need to be tagged for safety purposes..

7) Breaker # / Valve # - It could be a disconnect #, Contactor, Breaker, Valve…

8) Un-Tagged Position – Your normal operating position of the device.

9) Tag Position – The safe position the device is changed to so the work can be preformed safely.

10) Lock # - The number of the lock if used.

11) Authorized By – The supervisor who authorized the lock-out.

12) Tagged By – The person who changes the position of the device and puts the tag on the device.

13) Tag # - The sequential tag that was placed on the device.

14) Work Complete – The person who says the work is complete.

15) UnTag Code - The code that authorizes removal of tag and repositioning of devices.

            E – Electrical                         G – General                          M – Mechanical

16) Un Tagged By – The person who removes the tag and repositions the device.
